LinkedIn could have it’s own blog for bad UX. This looks like a normal list of actions to take for a the user’s profile I’m viewing. Except, I’m looking at their profile because they sent me an invitation. Sometimes I don’t remember who they are so I have to check out their profiles. Yet when I get to their profiles, there’s no confirm connection link. If I click ‘Add to network’ it goes through the process from scratch, but there’s already a pending request!

Update: Slight retraction, on some click paths there is a big message box prompting to confirm connection, but it doesn’t show up all the time (like when clicking from the Inbox preview on the home page).
